After the release of Street Fighter Sega realized that their console needed a controller with more than 3 buttons so along came the smaller and more comfortable to hold Mega Drive six button controller.  Arguably the best 16 bit controller ever made even though I personally loved the ASCII Super Famicom pad.  As well as having 6 action buttons and a start button the six button controller also has a MODE button on the top right shoulder.  The point of this button was to make the pad compatible with all Mega Drive software.  You see, some games wouldn't work with the controller or do crazy things.  So when you switched on a game that wasn't compatible with this pad and held down the MODE button then all would be fine.  Actually that wasn't 100% true.  There are 1 or 2 games out there that just will not work with this controller but do you really care about that?  I know I don't.  I'd say about 98.5% of games will work fine.
